Summer Holiday Club

Please tell us the following information 

- I would like to book my child in on the following days:

- I have paid for membership? Yes or no (tell us if you are a member who pays monthly Gold, or yearly Silver) 

- I have considered the pay scales ans the needs of my child (whether they can access the sessions imwith a support ratio of 1:5 or would benefit from 1:2 or 1:1 support) and am able to pay £..... for each day. 

- I will aim to arrive between 3.30-4 as I would love to sit with a member of the team and my child to share moments from the day together with videos  

- I would like to book some personalised sessions for my child to develop skills during their day (please provide details, for example an hour or Neurodevelopmental therapy weekly, 1 music therapy session on a Monday, a session with the speech and language therapist on Thursdays please).
